If you are a new patient to our office, the attached file contains our new patient forms that will need to be fille out when you arrive at our office.
APPOINTMENTS AND CANCELLATIONS
We try to schedule appointments at your convenience and at our availability.
We strive to see you and provide care for you efficiently and timely.
We respect your time and needs, so we block sufficient time to provide the best care for you.
Because of this, we ask that you kindly give us at least 24 hours’ notice if you have an emergency and must cancel your appointment, so we may contact other patients who might be on a waiting list to receive treatment.
We encourage parents to accompany their child to the initial exam and subsequent periodic recalls and preventive appointments.
We discourage parents to accompany their child to the clinical area when restorative treatment is to be done. It has been our experience that children behave better and are far more cooperative without the parent in the treatment room. This allows us to perform treatment more efficiently and effectively.
UPDATING PERSONAL INFORMATION
We ask that you inform us of any changes : Name, Phone numbers, Email address, and Mailing address as soon as you arrive, so we can keep your records up to date.
We ask that you keep us informed of any changes in Medical History, Medications, Allergies in order to modify your treatment if need be.
We currently accept most dental insurance providers.
Please bring your insurance information on your first visit.
As a courtesy to you, we will file the insurance claims.
For your information, insurance policies usually cover only a portion of the total treatment cost.
We will assist you in estimating your portion of the cost of treatment, but we cannot guarantee what your insurance will or will not cover. Therefore, you are ultimately responsible for the claim.
It is imperative that you keep us informed of ANY insurance changes.
